Service Overview

Case Management is a service that is responsive and flexible to the needs of each child and family. Each child and his/her family are assisted in gaining timely access to needed medical, psychiatric, therapeutic, social, educational and other supports.

The core of the case management process is teamwork and trust. Case Managers work in partnership with families in order to develop goals that will enhance a child’s potential and quality of life in the community. Case managers assess the child’s needs, develop plans to meet those needs, help the family find services near where they live, navigate school and educational services, monitor the child’s progress, provide follow-up with family to ensure needs are met, and advocate for services and supports.

Eligibility Criteria
  • Children under the age of 18 who have a diagnoses of intellectual or developmental disability
  • Children 17 and under who have a diagnosis of a serious mental health problem; have problems in personality development and social functioning; or need significant intervention for their needs from more than one agency
  • Children under the age of 7 who have different behaviors or maturity than most children of the same age; or have been exposed to physical and psychological stress
  • Children under the age of 6 who have a diagnoses of or are determined to be at risk of intellectual or developmental disability
